  • In order for any medication to be administered at school, A Parent/Physician Medication Request/Authorization Form must be completed and delivered to allow the school nurse to administer any prescription medication (s) you feel are necessary for your child's wellbeing during school hours. For example, the forms are used for medications to treat chronic headaches, asthma, allergies, diabetes, or other chronic conditions.
  • The form must be signed by both you and the physician or other health care provider. For your convenience, the school nurse may fax forms to the child's health care provider for signitures.
  • One form should be completed for each individual medication, per child, and must be updated each school year. Please let the school nurse know if you need extra forms.

If you would like your student to be able to take OTC (over the counter) medications at school, we ask that you complete and return the OTC medications form and return them to your child's school nurse as soon as possible. The parent request form will allow the school nurse to give your child some basic OTC medications as needed at school. It is optional. This consent form will only be good for ONE SCHOOL YEAR.

Request for OTC medications form: Please initial in the right hand column box for each medication that you would be willing to allow the school nurse to administer to your child. Only the OTC medications that you consent to may be administered to your child. Also, please note the following:
PARENT/GUARDIAN SIGNATURE IS REQUIRED AT THE BOTTOM OF THIS FORM.


If your child's health care provider does not work out of one of the offices listed that we have received standing orders from, you must have him or her sign the bottom of the form for authorization.  



You should be aware that any medication given to your child could possibly cause an undesirable side effect. If you have any questions about such side effects, please consult your child's physician or call the school nurse at 642-2596 ext. 153.
