Registration Forms

New Student Enrollment Procedures

In order to process your child(ren)'s enrollment, the following items must be provided at the time of registration:

=Birth Certificate

=Immunization Record

=Photo ID of Parent (Driver License, etc.)

=Proof of Residency and Moving Permit (example:  Deed, Lease, Utility Bill)

=Current ER, NOREP, and IEP (Special Education documents, if applicable)

=Custody Orders (if applicable)

Once the registration is completed online you will need to schedule an appointment to personally meet with our staff to sign the forms and provide the above required documents.  Please call Gabrielle Elsasser at 610-377-4490, ext. 1536 to schedule the appointment.
