Welcome!!!  If you are ever in the Elementary building, please stop and see the nurse's suite located behind the main office. I would like to remind everyone to please keep your child's emergency information updated. This includes emergency contacts, phone numbers, addresses, and health conditions. I cannot stress enough the importance of keeping this information current.

If your child requires medication be given during the school day, please have the district's medication permission form completed by the prescribing physician. Over-the-counter medications not listed on the emergency card as standing orders also reqire a medication permission form be completed by the prescribing physician. Medications must be delivered to the school by a parent/guardian in the original container. Medication permission forms can be accessed on the district's or my website. A new medication order is required at the start of each new school year. 

If your child has a history of seizures, asthma, or an allergy, new action plans must be submitted for the current school year. Forms will be sent home with all students who have these conditions indicated on their emergency cards.
