noun 1. movement, passage, or change from one position, state, stage, subject, concept, etc., to another; change: the transition from adolescence to adulthood.
 
 
This site features resources and documents that are beneficial to students as they transition from school to adulthood (work, training, independent living, etc.).
 
 
As a student who has a disability transitions from a school setting, where services have been provided through IDEA - where a student has an IEP and specially designed instruction and is supported automatically (entitlement) - they move to adulthood and post-school outcomes where the student is now has the opportunity to access supports under ADA Law (Americans with Disabilities Act) - moving from a place of entitlement to a place of advocacy and disclosure. 
 
As students move from high school where they may have had supports provided for them automatically to a college or additional schooling, they will now need to disclose their disability and ASK for the assistance that they need, often times explaining why they need the accommodations they are requesting.
 
In employment, students may now need to receive training or be asked to do tasks that in a school setting they would have had someone to help them with, they may now need to ADVOCATE for what they need and MUST know their rights.
 
As students transition from school to work or further education and independent living there are many new skill sets that they need to have and much information they must have access to - these may include:
  • Understanding and being able to explain their disability
  • Obtaining a new social security card
  • Obtaining a birth certificate
  • Earning a Learner's Permit or Driver's License
  • Accessing public transportation
  • Accessing food banks and support
  • Accessing mental health services
  • Applying for housing assistance
  • Applying for medical assistance or MAWD
