Transition is a process that we all go through at different times in our lives.  As young children, we transition from constant care to beginning of independence, we transition from the home to school, from the end of a school year to the summer months, and back to school again in the fall.  Sometimes we transition from different homes, and different school settings. Some transitions are large and others are very small. For some individuals these transitions, no matter how small can be very difficult, but for others these transition periods can go smoothly.  For individuals with disabilities, transitions related to the move from high school to post-school outcomes can be met with barriers.  The change from a place of academic, physical, and/or behavioral support to a place of minimal support can present difficulties that can create barriers to employent, to independent living, to college, etc. 

It is the hope of this site, to provide a variety of resources, answers, and supports for those who have disabilities, those who provide support for those with disabilities, and those who work in the disability world...within the Snyder, Union, and Northumberland Counties.

Unfortunately, each time I discover a new resource, I am met with another 10 resources that I didn't know about previously. This is good! (and frustrating, because I think this site will continue to grow and develop for sometime). 

The visual of the site is very limiting, but I have done my best to make it possible to be navigated....you will find some information listed in multiple locations (medical assistance for instance).  If you discover links that don't work, please let me know. If you discover information you don't agree with or have questions about, please let me know as well.  If you have suggestions for other resources, I would love to hear from you as well. 

Over the next few weeks I will attempt to add resources to this site that I have not yet added. I am concerned about it becoming unmanageble for those visiting, please let me know if information needs to be simplified or removed. I appreciate your input...I do not hold the knowledge of these sources, but I have discovered there is so much out there and there is so much to be learned and having a common place to discover this information is helpful.
